Job Title: Community Nursing Specialist Practitioner

We are seeking an experienced Community Nursing Specialist Practitioner to join our team at Manchester University NHS Foundation Trust. As a key member of our community nursing team, you will be responsible for providing high-quality, patient-centered care to adults in their own homes and community settings.

The successful candidate will have a strong background in community nursing, with experience in leading and coordinating nursing teams. You will work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care and coordination.

About the Role

This is a full-time permanent position, working 37.5 hours per week. The successful candidate will be required to work variable shift patterns, including evenings and weekends. A full driving license and access to a car are essential for this role.

You will be responsible for:

  • Providing high-quality, patient-centered care to adults in their own homes and community settings
  • Leading and coordinating nursing teams to deliver effective patient care
  • Working closely with multidisciplinary teams to ensure effective patient care and coordination
  • Developing and implementing care plans to meet the needs of patients
  • Monitoring and evaluating patient outcomes to inform care delivery

We are looking for a highly motivated and experienced nurse who:

  • Has a strong background in community nursing
  • Has experience in leading and coordinating nursing teams
  • Has a good understanding of patient-centered care and the principles of community nursing
  • Has excellent communication and interpersonal skills
  • Is able to work independently and as part of a team
