About the Role

We are seeking a highly skilled and experienced Community Specialist Practitioner to join our team at Manchester University NHS Foundation Trust. As a key member of our community nursing service, you will play a vital role in delivering high-quality care to patients in their own homes and community settings.

Main Responsibilities
  • Lead and coordinate nursing teams to provide complex nursing care to adults in their own homes and community settings.
  • Work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care and coordination.
  • Develop and implement clinical skills and knowledge to improve patient outcomes and service delivery.
  • Contribute to the development of new and existing clinical services, ensuring they meet the needs of patients and the community.
Requirements
  • Registered Nurse with a Community Specialist Practitioner qualification.
  • Experience of working in community nursing, with a strong understanding of the complexities of community care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to travel throughout different locations, with a flexible approach to meet the needs of the service.
