About the Role

We are seeking an experienced Community Nursing Specialist Practitioner to join our team at Manchester University NHS Foundation Trust. As a key member of our community nursing service, you will play a vital role in delivering high-quality care to patients in their own homes and community settings.

Key Responsibilities
  • Lead and coordinate nursing teams to provide complex nursing care to adults in their own homes and community settings.
  • Work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care.
  • Develop and implement clinical skills and knowledge to improve patient outcomes.
  • Collaborate with the Band 7 team leader to ensure quality care delivery and implement audit recommendations.
Requirements
  • Registered Nurse with a Community Specialist Practitioner qualification.
  • Experience of working in community nursing and leading teams.
  • Ability to travel throughout different locations and work flexible hours.
  • Computer literate and able to utilize electronic records/systems.
