Dementia Coordinator Role at Woodlands Care Home

We are seeking a compassionate and skilled Dementia Coordinator to join our team at Woodlands Care Home in Aberdeen. The ideal candidate will have experience working with individuals with dementia, strong communication skills, and a proactive approach to supporting residents and their families.

Key Responsibilities:
  • Develop and implement tailored support plans for residents, reflecting their individual needs, preferences, and interests.
  • Collaborate with residents and their families throughout the moving-in process, ensuring a smooth transition and a welcoming environment.
  • Design and coordinate well-being programmes and activities that align with residents' hobbies and routines, enhancing their daily experience.

About You:

  • You should have experience working with individuals with dementia in a care setting, with strong communication skills and a compassionate approach to supporting families and residents.
  • A proactive, flexible mindset, and the ability to work collaboratively with staff, residents, and external professionals is essential.
