At Woodlands Care Home, we are seeking a compassionate and skilled Dementia Coordinator to join our team.

Job Summary:

We are looking for an individual with experience working with individuals living with dementia in a care setting. The successful candidate will have strong communication skills and a proactive approach to supporting families and residents.

The role involves collaborating with residents and their families to develop tailored support plans, designing and coordinating well-being programmes, and liaising with external professionals to improve quality of life.

Key responsibilities include:

  • Developing person-centred support plans that reflect the individual needs, preferences, and interests of residents
  • Collaborating with families to ensure a smooth transition into care
  • Designing and coordinating well-being programmes and activities that enhance daily experiences
  • Liaising with families to identify and implement effective strategies for managing dementia-related behaviours

The ideal candidate will be passionate about enhancing the quality of life for people living with dementia and possess excellent communication and interpersonal skills.

We offer a competitive salary of £13.00 per hour, along with a range of benefits including pension, onsite parking, and paid PVG certification.
