About the Role

We are seeking a highly skilled and experienced Adult Mental Health Practitioner to join our team at Sheffield Health and Social Care NHS Foundation Trust. As a key member of our Primary and Community Mental Health Service, you will play a vital role in providing high-quality care and support to individuals with severe mental illness.

Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments to identify individuals' mental health needs and develop personalized care plans.
  • Provide direct specialist mental health psychosocial interventions to adults and older adults with severe mental health needs, promoting recovery and reducing symptoms.
  • Collaborate with multidisciplinary teams, including primary care settings, to ensure seamless care coordination and effective service delivery.
  • Develop and maintain effective working relationships with colleagues, stakeholders, and community partners to promote mental health awareness and education.
  • Participate in clinical audit, team standards, trust policies, and clinical standards development to ensure high-quality care and service improvement.
Person Specification
  • Evidence of continuous professional development in mental health nursing, social work, or occupational therapy.
  • Evidence of higher practice skills in mental health care across the age range.
  • RNMH with current registration with NMC/Social Work Qualification/Occupational therapy.
Desirable
  • Post-reg study to degree or post-grad diploma level.
  • Experience in delivering training to nurses and other disciplines.
Knowledge and Skills
  • Excellent risk assessment and management skills.
  • Experience of working in community mental health care settings.
