Job Title: Adult Mental Health Practitioner

Job Summary:

We are seeking a highly skilled and dedicated Adult Mental Health Practitioner to join our team at Sheffield Health & Social Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality mental health services to adults with severe mental illness.

Key Responsibilities:

  • Provide direct specialist mental health psychosocial interventions to adult and older adult service users with severe mental health needs.
  • Develop and maintain effective working relationships within the multidisciplinary team, representing your professional background as a mental health practitioner.
  • Provide specialist psychosocial assessment and advice and consultation on clients' mental health and wellbeing with colleagues from Primary care settings and within your multidisciplinary team.
  • Carry out procedures in accordance with best practice and within your own skill set or under supervision of others.
  • Maintain accurate records about the care and treatment provided.
  • Review and evaluate professional standards in conjunction with the quality Team.
  • Ensure that the developing requirements of the Mental Health Nursing/social work/Occupational therapy profession are understood by the wider team.
  • Ensure that clinical, supportive and therapeutic interventions protect the rights of the patient and are in accordance with evidence-based practice and Primary and community mental health service policies.

Working Arrangements:

The Primary and Community Mental Health Service is delivered in a mix of on-site and at-home working (virtual appointments). The mental health practitioner role is expected to see 26 appointments each week with 5 new patients each week. Patients are to be seen either weekly or bi-weekly, with future booked appointments booked well in advance.

Requirements:

  • Knowledge of working with individuals who experience suicide, self-harm, acute mental illness presentations, and dementia/confusion/delirium.
  • Collaborate with others in the development of education opportunities encouraging participation of all under your supervision.
  • Participate in the development and implementation of clinical managerial and educational systems within the service to enhance the continual development of the team.
  • Actively promote the development of the team and a culture of education, training, and research within the service.
  • Ensure clinical supervision is provided to all staff within your area of supervisory responsibility.
  • Ensure that learning objectives for students are identified and that programmes are devised to enable their achievement.
  • Promote insight and understanding of mental health to other health workers and agencies in the community.
  • Encourage Health Promotion with all staff, carers, and patients.
  • Participate in and contribute to the development of clinical audit, team standards, trust policies, and clinical standards.
