Senior Care Practitioner

As a Senior Care Practitioner with Home From Home Care Limited, you will play a vital role in providing person-centered care and support to individuals with complex needs. Your primary responsibilities will include providing high-quality care and support, building strong relationships with service users and their families, and collaborating with our multidisciplinary team to deliver exceptional care.
Responsibilities:
- Provide individualized care and support to service users, taking into account their unique needs and preferences.
- Work collaboratively with other healthcare professionals to develop and implement effective care plans.
- Engage with service users and their families to understand their needs and preferences, and to provide them with information and support.
Requirements:
To succeed in this role, you will need:
- A minimum of 2 years' experience working in a care environment.
- An understanding of the principles of person-centered care and the importance of delivering high-quality care.
- Excellent communication and interpersonal skills, with the ability to work effectively with people from diverse backgrounds.
Compensation:
The estimated annual salary for this position is £30,000 - £40,000, depending on experience.
