Key Responsibilities:
  • Assess and identify the needs of service users
  • Develop and implement person-centred plans to meet the needs of service users
  • Monitor and evaluate the effectiveness of support plans

As a Community Care Worker with Creative Support Ltd, you will receive comprehensive training and the opportunity to work towards a QCF Diploma in Health and Social Care. We offer a competitive salary of £23,500-£26,000 per annum, depending on experience, plus benefits including pension contributions, 28 days' annual leave, and enhanced DBS check.
