About the Role

We are seeking a highly motivated and experienced Rehabilitation Assistant to join our team at Central and North West London NHS Foundation Trust. As a Rehabilitation Assistant, you will play a vital role in supporting our patients to achieve their rehabilitation goals and improve their overall quality of life.

Key Responsibilities
  • Assist in the delivery of rehabilitation programs to patients with complex physical needs
  • Work closely with our therapy teams to develop and implement individualized treatment plans
  • Provide support and guidance to patients and their families to promote independence and self-management
  • Contribute to the development and maintenance of a safe and supportive environment for patients
  • Collaborate with other healthcare professionals to ensure seamless care coordination
Requirements
  • NVQ 3 Health and Social Care or equivalent qualification
  • Experience working in a health and social care setting, preferably in a rehabilitation role
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Full and current UK driving license and access to a vehicle for work
