We are seeking a skilled Physiotherapist to join our team in a community-based setting, working with adults to provide rehabilitation services.
Key Responsibilities- Assess and treat clients with diverse needs, using evidence-based practices and patient-centered principles.
- Manage a caseload of clients with simple to complex needs, with supervision and guidance as necessary.
- Develop and implement individualized treatment plans, working closely with multidisciplinary teams.
- Screen and assess complex referrals, determining priority, needs, and initiating intervention plans.
- Collaborate with other healthcare professionals to ensure effective referral management and best client care.
- Prescribe, delegate, and monitor the work of rehabilitation assistants and counter-sign documentation.
- Educate and teach carers/health workers to participate in ongoing rehabilitation care programs.
- Contribute to the implementation of new specialist clinical services within the team.
- Maintain and update a Continuing Professional Development Portfolio (CPD).
