About the Role

This is an exciting opportunity to join our team as a Care Coordinator, working closely with individuals to develop innovative support plans that help them achieve their goals and aspirations. You'll work collaboratively with colleagues, families, and other stakeholders to create personalized care packages that cater to each person's unique needs.

As a Care Coordinator, you'll be responsible for:

  • Conducting assessments to identify individual needs and strengths
  • Developing and implementing personalized care plans that promote independence and inclusion
  • Collaborating with healthcare professionals, social workers, and other agencies to ensure seamless care delivery
  • Providing emotional support and guidance to individuals and their families
