About the Job

We are seeking a highly skilled Aged Care Services Coordinator to join our team at Centacare North Queensland.

In this role, you will be responsible for conducting face-to-face comprehensive assessments for clients seeking aged care services.

The successful candidate will have strong clinical expertise in managing complex cases, identifying when additional specialist interventions or re-ablement strategies are required.

You will work closely with a multi-disciplinary team of nurses, allied health professionals, and medical experts to ensure clinical input where necessary, especially in complex cases.

The estimated salary for this position is $95,000 - $115,000 per annum, depending on experience.

Key Responsibilities:

  • Conduct face-to-face comprehensive assessments for clients seeking aged care services
  • Work collaboratively with a diverse range of stakeholders, including healthcare professionals, community organisations, and government agencies
  • Develop and implement individualised care plans to meet the needs of clients
  • Provide clinical expertise in managing complex cases, identifying when additional specialist interventions or re-ablement strategies are required
  • Engage with external service providers to ensure accurate referrals and access to complementary supports
