  • About the role
    The Dementia Support Specialist will be responsible for providing high-quality care and support to patients with dementia and delirium in a hospital setting.
  • Main duties and responsibilities

    • Organise and plan patient caseloads in consultation with senior team members to meet service and patient priorities.
    • Ensure that own practice meets Trust standards and aligns with Trust policies.
    • Attend courses, clinical supervision, and peer supervision to maintain ongoing personal and professional development.
    • Comply with Trust policies and procedures.
    • Utilise information and digital technology systems to contribute to record-keeping and data analysis.
    • Work with patients to improve mental health, physical health, and wellbeing through therapeutic activities.
    • Support ward staff in managing patients with behavioural and psychological symptoms of dementia.
    • Identify patients with dementia or delirium and proactively make contact with them and their families/carers.
    • Ensure appropriate documentation and flagging are in place and make referrals as necessary.
    • Liaise with wards to ensure 'What matters to me' documents are completed meaningfully.
