We are seeking a highly skilled Dementia Specialist Care Lead to join our team at NHS. This is an exciting opportunity to make a real difference in the lives of individuals affected by dementia and their families.

The successful candidate will have significant post-registration clinical experience supporting people with dementia and their carers/families, as well as experience in completing holistic nursing assessments, providing bio-psychosocial interventions, and facilitating training/development of staff. They will also possess strong leadership skills, including the ability to influence change, act as a decision maker and advocate, and use appropriate information technology.

In this role, you will be responsible for delivering regular clinical 1:1 sessions for families affected by dementia, providing specialist nursing assessment, and working collaboratively with other professionals to improve the identification and management of co-morbidities and frailty. You will also develop and support care plans for people with dementia and families, ensuring the safeguarding of vulnerable adults and promoting their rights and best interests.
