Advanced Social Work Practitioner

We are seeking a dynamic and passionate Social Worker to join our team in Buckinghamshire Council. As an Advanced Practitioner, you will play a key role in supporting individuals and families to achieve their goals and improve their wellbeing.
As a member of our South Locality Team, you will work closely with colleagues and external partners to deliver high-quality services that meet the needs of our residents. You will be responsible for carrying out needs assessments, creating person-centred care plans, and ensuring the delivery of services that promote independence and self-determination.

Key Responsibilities- Carry out needs assessments and create person-centred care plans that promote independence and self-determination
