About the Role

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at Chiltern House Medical Centre. As a key member of our clinical team, you will be responsible for delivering high-quality patient care and contributing to the development of our clinical services.

Key Responsibilities
  • Provide comprehensive clinical care to patients, including health promotion, chronic disease management, and health prevention.
  • Support the management team in reviewing clinical policy and procedure.
  • Contribute to the development of clinical services and initiatives.
  • Collaborate with other healthcare professionals to ensure seamless patient care.
  • Maintain accurate and up-to-date patient records.
Person Specification
  • Registered Nurse with a postgraduate diploma or degree in Advanced Practice.
  • Experience working in a primary care environment, preferably as a practice nurse or community nurse.
  • Strong clinical knowledge and skills, including wound care, ECGs, venepuncture, and immunisations.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
