About the Role

The Advanced Clinical Practitioner will be responsible for delivering high-quality clinical services to NHS patients. This includes health promotion, chronic disease management, and supporting the management team in reviewing clinical policy and procedure.

Key Responsibilities:

  • Deliver clinical services to patients, including health promotion and chronic disease management.
  • Support the management team in reviewing and updating clinical policy and procedure.
  • Maintain accurate and up-to-date patient records.
  • Collaborate with other healthcare professionals to ensure seamless patient care.

Person Specification:

  • Registered Nurse qualification.
  • Postgraduate diploma or degree in Advanced Practice.
  • Experience working in a primary care environment.
  • Experience working as a practice nurse or community nurse.

Clinical Knowledge:

  • Wound care and removal of sutures and staples.
  • ECGs and venepuncture.
  • New patient medicals and chaperone procedure.
  • Requesting pathology tests and processing results.
  • Diabetes, asthma, and CHD management.
  • Immunisations and health promotion strategies.
