About the Role

We are seeking an experienced and skilled Community Health Specialist to join our team in East and Central Brighton. As a key member of our Primary Care Network, you will play a vital role in providing high-quality, patient-centered care to our community.

Key Responsibilities
  • Lead the delivery of holistic assessments, chronic disease management, and onward referrals to ensure seamless care for our patients.
  • Work closely with our multidisciplinary team to develop and implement health improvement plans, addressing priority areas and promoting evidence-based practice.
  • Provide expert clinical guidance and support to junior staff, ensuring they have the necessary skills and knowledge to deliver high-quality care.
  • Contribute to the development of clinical guidelines, policies, and procedures, ensuring they are embedded within our service and communicated to relevant personnel.
Requirements
  • Registered Nurse with a strong background in community nursing and experience in safeguarding pathways and risk management.
  • Non-medical prescribing or working towards advanced clinical practice.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
  • Ability to travel within the requirements of the role, with a valid driving license.
