Job Title: Part Time Health

About the Role:

We are seeking a skilled Behavioral Health Clinician to join our dynamic team at Charles River Community Health. As a key member of our outpatient team, you will provide high-quality care to patients with physical and behavioral health needs.

Key Responsibilities:

  • Provide brief interventions for physical and behavioral health problems.
  • Attend Behavioral Health and Chronic Care meetings to identify problems related to patient services and make recommendations for improvement.
  • Assist with outcome measures and data collection (when requested).
  • Facilitate referrals for clinically indicated services outside of the primary care clinic.
  • Perform other duties as requested by the Director of Behavioral Health or designee.

Requirements:

  • Bilingual in Spanish or Portuguese highly preferred.
  • Experience in mental health assessment/evaluation/diagnostic capabilities and crisis intervention/management.
  • Ability to travel between both Charles River Community Health sites required.
  • Experience working effectively and collaboratively with a broadly diverse staff and patient community.
