Job Summary

A rare opportunity has arisen within the Adult Community Learning Disability team for a Band 6 Community Practitioner to work within our Specialist Behaviour pathway.

Main Duties

Manage case work under the direction of a care coordinator/pathway clinical lead, providing ongoing assessment and recovery-focused care planning for service users with low risks. Provide effective treatment and intervention to service users with complex needs, requiring support from our Specialist Behaviour team. Deliver evidenced-based clinical and psychologically-based interventions, with appropriate training and supervision. Work as a member of the multi-disciplinary team, ensuring high-quality, individualized care that promotes recovery and wellbeing, and maximizes independence.

Person Specification
  • Professional qualification and current professional registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
  • Up-to-date knowledge/experience of relevant legislation.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
Desirable
  • Experience of working within a community team.
  • Non-Medical Prescribing.
  • Additional training/qualifications in positive behaviour support.
Knowledge and Experience
  • 12 to 24 months of band 5 qualified nurse experience, dependent on service need.
  • Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
Skills and Competencies
  • Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
  • Risk Assessment and Risk Management/Safety Planning skills.
  • Demonstrate an understanding of effective and appropriate treatment options, including a range of evidence-based clinical approaches.
  • Demonstrate effective written and verbal communication skills.
  • Evidence of a high level of professional, organisational, and leadership skills (self and others).
Desirable Skills
  • Clinical Leadership/delegation skills.
  • Ability to use electronic recording and relevant IT equipment.
  • Proven experience of acting as an effective advocate for service users and their carers.
