About the Role

We are seeking a highly skilled and experienced Community Practitioner to join our Adult Community Learning Disability team. As a key member of our Specialist Behaviour Pathway, you will play a vital role in providing high-quality, individualized care to individuals with complex mental health needs.

Key Responsibilities
  • Manage case work under the direction of a Care Coordinator/Pathway Clinical Lead and provide ongoing Assessment and Recovery-focused Care Planning for service users with low-risk needs.
  • Provide effective treatment and intervention to service users with complex and substantial needs, requiring support from our Specialist Behaviour team.
  • Deliver evidenced-based clinical and psychologically-based interventions, with appropriate training and supervision.
  • Work as a member of the multi-disciplinary team to ensure high-quality, individualized care is delivered, promoting Recovery and Wellbeing and maximizing Independence.
  • Provide compassionate care based on empathy, kindness, respect, and dignity.
Requirements
  • Professional Qualification and current Professional Registration as a First-Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
  • Up-to-date knowledge/experience of relevant legislation.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
Desirable Qualifications and Experience
  • Experience of working within a community team.
  • Non-Medical Prescribing.
  • Additional training/qualifications in Positive Behaviour Support.
Skills and Competencies
  • Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
  • Risk Assessment and Risk Management/Safety Planning skills.
  • Demonstrate an understanding of effective and appropriate treatment options, including a range of evidence-based clinical approaches.
  • Demonstrate effective written and verbal communication skills.
  • Evidence of a high level of professional, organizational, and leadership skills (self and others).
