About the Role

We are seeking a skilled Community Practitioner to join our Adult Community Learning Disability team, working within our Specialist Behaviour pathway. As a key member of our multidisciplinary team, you will provide high-quality, individualized care to service users with complex needs, promoting recovery and wellbeing.

Main Responsibilities

As a Community Practitioner, you will be responsible for managing case work under the direction of a care coordinator/pathway clinical lead, providing ongoing assessment and recovery-focused care planning for service users with low-risk needs. You will also deliver evidence-based clinical and psychologically based interventions, with appropriate training and supervision, to service users with complex and substantial needs requiring support from our Specialist Behaviour team.

Requirements

  • Professional qualification and current professional registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist
  • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post
  • Up-to-date knowledge/experience of relevant legislation
  • Proven track record of post-registration clinical experience relevant to the post
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection

Desirable Qualifications and Experience

  • Experience of working within a community team
  • Non-Medical Prescribing
  • Additional training/qualifications in positive behaviour support
