The Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust offers an exceptional opportunity for a dedicated individual to join our team in delivering high-quality care to individuals with complex mental health needs.

This role involves managing casework under the direction of a Care Coordinator/Pathway Clinical Lead, providing ongoing Assessment and Recovery-focused Care Planning for service users with low-risk needs, and delivering evidence-based clinical and psychologically based interventions.

The successful candidate will have a proven track record of post-registration clinical experience, excellent written and verbal communication skills, and a strong understanding of professional accountability and NMC standards.

The ideal candidate will possess excellent leadership and delegation skills, be able to work effectively in a multidisciplinary team, and demonstrate a commitment to delivering high-quality care that promotes recovery and wellbeing.

To succeed in this role, you will need to have a first-level registered nurse qualification, current professional registration, and evidence of post-registration training in clinical/psychological interventions. You should also have a proven ability to carry out comprehensive written assessments, risk assessment and risk management skills, and effective treatment options knowledge.

As a Community Practitioner, you will play a vital role in supporting individuals with complex mental health needs, working collaboratively with colleagues to deliver high-quality care that meets the unique needs of each service user.
