Community Practitioner

A rare opportunity has arisen within the Adult Community Learning Disability team for a Band 6 Community Practitioner to work within our Specialist Behaviour pathway.
Main DutiesManage case work under direction of care co-ordinator/ Pathway Clinical Lead and provide ongoing Assessment and Recovery focused Care Planning for service users who do not have enhanced needs and where risks are assessed as low.
Provide effective treatment and intervention to service users with complex and substantial needs requiring support from our Specialist Behaviour team.
Deliver evidenced based clinical and psychologically based Interventions, with appropriate Training and supervision and as directed by Pathway Clinical Lead and post holder will work as a member of the multi-disciplinary team ensuring that high quality, individualised care is delivered and that this promotes Recovery and Wellbeing and maximises Independence.
Provide compassionate care that is based on empathy, kindness, respect and dignity.

Person SpecificationEducation and Qualification
- Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
- Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
- Up-to-date knowledge/experience of relevant legislation
- Proven track record of post registration clinical experience relevant to the post.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
Desirable
- Experience of working within a community team
- Non-Medical Prescribing
- Additional training/qualifications in positive behaviour support
Knowledge and Experience
- 12 to 24 month band 5 qualified nurse experience dependent upon service need
- Awareness and understanding of professional accountability, NMC standards and other local and national initiatives
Skills & Competencies
- Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs
- Risk Assessment and Risk Management / Safety Planning skills
- Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in Guidance)
- Demonstrate effective written and verbal communication skills
- Evidence of a high level of professional, organisational and leadership skills (self & others)
Desirable
- Clinical Leadership/delegation skills
- Ability to use electronic recording and relevant IT equipment
- Proven experience of acting as an effective advocate for service users and their carers
