Job Summary

A rare opportunity has arisen within the Adult Community Learning Disability team for a Band 6 Community Practitioner to join our team working within our Specialist Behaviour pathway.

Main Duties

To manage case work under direction of care co-ordinator/ Pathway Clinical Lead and provide ongoing Assessment and Recovery focused Care Planning for service users who do not have enhanced needs and where risks are assessed as low, under the supervision of the Pathway Clinical provide, under the direction of a Care Coordinator and or Pathway Clinical Lead, effective treatment and intervention to service users with complex and substantial needs requiring support from our Specialist Behaviour deliver evidenced based clinical and psychologically based Interventions, with appropriate Training and supervision and as directed by Pathway Clinical Lead and post holder will work as a member of the multi-disciplinary team ensuring that high quality, individualised care is delivered and that this promotes Recovery and Wellbeing and maximises Independence. To provide compassionate care that is based on empathy, kindness, respect and dignity.

Person Specification
  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Up-to-date knowledge/experience of relevant legislation
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
