Care Coordinator for Adults with Learning Disabilities

We are seeking a skilled Care Coordinator for Adults with Learning Disabilities to join our team at The Mungo Foundation. As a member of our team, you will play a vital role in providing exceptional care and support to adults with learning disabilities and associated health issues.
Your primary responsibility will be to coordinate and deliver high-quality care services, ensuring that our service users receive the support they need to live fulfilling lives. This will involve working closely with our service users, their families, and other professionals to develop and implement individualized care plans.
You will also have the opportunity to work on short breaks, providing individuals with the chance to enjoy time away with friends in various locations across Britain. In this role, you will be responsible for:
- Coordinating and delivering high-quality care services
- Developing and implementing individualized care plans
- Working closely with service users, families, and other professionals
- Providing support for daily living tasks, social activities, and practical tasks
- Participating in shift work, including sleepovers, and traveling to various locations
We estimate that the salary for this position is around £25,000-£30,000 per year, depending on experience.
