About the Role

We are seeking a highly skilled and experienced Clinical Nurse Specialist to join our team at Sciensus Pharma Services Limited. As a key member of our multi-disciplinary team, you will be responsible for delivering high-quality, patient-centered care to individuals in their own homes.

Key Responsibilities
  • Deliver effective and high-quality nursing care to patients in a home environment, facilitating early supported transfer from hospital or preventing unnecessary admissions.
  • Work collaboratively with patients, stakeholders, and patient support networks to support treatment and recovery.
  • Accurately document all patient care and clinical observations to provide real-time data for business and patient goals.
  • Build lasting relationships with patients and stakeholders, ensuring seamless care coordination and communication.
Requirements
  • Registered General Nurse (RGN) with experience in an acute care setting, including independent clinical decision-making and supervising others.
  • Strong understanding of therapies and drugs relevant to our service offering, including NHS knowledge for managing patients with treated conditions.
  • Excellent communication and interpersonal skills, with the ability to communicate sensitively and effectively with patients and stakeholders.
  • Adaptable and flexible, with the ability to cope with changing service demands.
