About the Position

We are seeking a Care Coordinator for Young Adults to join our team in Ealing. This is a one-year fixed-term position, with the possibility of extension. The successful candidate will work with young adults aged 18-25 who have experienced mental health-related hospital admissions.

As a Care Coordinator, you will be responsible for providing holistic assessments, conducting regular key work sessions, and developing support plans that cater to each individual's unique needs. You will also promote self-management and empowerment through informed consent and decision-making.

This role offers a competitive salary of £28,500 per annum, along with a range of benefits including 25 days' annual leave, pension schemes, staff discounts, healthcare, and career progression opportunities.

About the Team

Oaklands & St Kilda is a pair of supported accommodation services providing mental health rehabilitation to young adults. Our team works collaboratively to create a welcoming and supportive environment, promoting independence and empowering individuals to manage their mental health.

Your Key Responsibilities
  • Build trusting relationships with young people and create a positive atmosphere
  • Carry out comprehensive assessments and develop tailored support plans
  • Conduct regular key work sessions, focusing on goal achievement and empowerment
  • Ensure ongoing risk assessment and management, embracing a 'positive risk-taking' approach
  • Support young people in accessing relevant services and advocating for their rights
  • Promote self-management and empowerment through informed consent and decision-making
  • Develop productive relationships with external stakeholders to enhance service outcomes
About You

We are looking for an individual with a passion for supporting young adults with mental health challenges. The ideal candidate will possess excellent communication skills, a non-judgmental attitude, and a commitment to creating a safe and supportive environment.

To succeed in this role, you should have:

  • A minimum of NVQ Level 2/3 or equivalent experience supporting young people or individuals with mental health challenges
  • Specialist knowledge relevant to supporting young adults with mental health diagnoses
