About the Role

We are seeking a qualified and experienced professional to join our Integrated Care Team within primary care, supporting individuals with long-term physical and mental health issues to maximize their potential and well-being.

The successful candidate will have a minimum of two years' experience working within community mental health services and possess the ability to work autonomously within a primary care community setting.

A full and valid driving licence and access to a vehicle for work purposes are essential due to the community element of this role.

Main Responsibilities

Working within Primary Care, you will facilitate mental health/cognitive assessments and formulate diagnosis, treatment/post diagnostic plans with the supervising Consultant Psychiatrist.

You will favour a person-centred, psychosocial approach to practice, enabling independence and avoiding hospital admissions where possible.

Excellent risk assessment, communication, and documentation skills are essential, along with a willingness to problem solve and signpost to other services, where appropriate.

  • Professional Qualification in Mental Health Nursing or Occupational Therapy
  • Non-medical prescribing
Requirements
  • Experience of providing brief psychosocial interventions with adult/OA mental health patients
  • Experience of conducting MH assessment/Memory assessments
  • At least two years experience working in a community setting
