Job Responsibilities

You will be working within a dynamic multi-disciplinary team to provide community rehabilitation services to the Mental Health and Learning Disability Division for the areas of North Wales.

Key responsibilities include:

  • Providing intensive community rehabilitation services to service users with complex emotional needs and complex rehabilitation requirements.
  • Working closely with the multidisciplinary team to develop and implement individualized care plans.
  • Collaborating with other healthcare professionals to ensure seamless delivery of care.

Estimated Salary: £45,754 - £60,595 per annum
