About the Role

We are seeking a highly skilled and experienced Nurse Practitioner to join our team as a Triage and Assessment Professional. As a key member of our urgent care team, you will be responsible for providing high-quality clinical decision-making and assessment services to patients across Wales.

Key Responsibilities
  • Undertake telephone triage assessments of patients in an efficient and effective manner
  • Advise on the most appropriate intervention to meet the needs of the patient, using best clinical practice and adhering to Trust Policy, protocols, and guidelines
  • Work collaboratively with a multi-disciplinary team, including GPs, Pharmacists, and Mental Health Practitioners, to provide comprehensive care to patients
  • Develop and maintain strong relationships with patients, families, and carers to ensure seamless care coordination
  • Participate in continuous professional development to stay up-to-date with the latest clinical knowledge and best practices
Requirements
  • NMC Registered Nurse (adult or child) or HCPC Registered Paramedic with relevant clinical experience
  • Strong listening and communication skills, with the ability to empathize with and reassure patients and their families
  • Ability to work independently and as part of a team, with a collaborative and flexible approach
  • High levels of current clinical knowledge, with a commitment to ongoing training and development
  • Excellent time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities
