Adult Community Support Practitioner

We are recruiting for an Adult Community Support Practitioner to work within our community mental health team. As a skilled practitioner, you will be responsible for assessing and developing individualized care plans to support individuals with mental health needs.
Your primary goal will be to empower individuals to develop independent living skills and live in the community. To achieve this, you will work collaboratively with individuals, families, and caregivers to identify strengths and areas for development, and develop tailored strategies to address these needs.
The successful candidate will possess a recognized professional qualification in registered nursing, social work, or occupational therapy, with post-registration experience working within adult mental health. You will also have management/leadership training and experience managing and developing staff.
