Job Title: Senior Social Worker Adults

SW Locums is seeking a highly skilled and experienced Senior Social Worker to join our team in Adult Hospital Services. As a Senior Practitioner, you will be responsible for assessing, managing, and delivering care to patients allocated to you.

You will work closely with the clinical team leader and support your peers to ensure effective team collaboration and decision-making. Your commitment to collaborative working and liaison with other health and social professionals is essential.

Key Responsibilities:
  • Organize and prioritize your workload to meet deadlines and ensure effective risk management.
  • Deputize for the Clinical Lead in their absence and maintain organizational knowledge of Trust protocols and procedures.
  • Provide accurate records of information required by the Trust for audit purposes.
  • Ensure effective risk management at team level by accident/incident reporting, assessing, and controlling risk.
Requirements:
  • Demonstrate ability to work under pressure, prioritize workload, and meet deadlines.
  • Experience of working across health and social care boundaries.
  • Sound knowledge of social care processes and responsibilities in line with 'Personalisation'.
  • Good decision-making practice, prepare for, and participate in decision-making forums.
  • Knowledge of relevant medical conditions and their impact on service users and carers.
  • Ability to assess and manage risk, balancing rights and responsibilities of service users and carers.
  • Professional assertiveness to justify decisions and uphold professional social care practice, values, and ethics.
  • Flexibility in thinking, openness of mind, and readiness for change.
  • Understanding of safeguarding standards that underpin safeguarding activity, including assessment of Mental Capacity.
