Hamilton Woods is seeking a highly skilled and experienced individual to fill the role of Care Services Coordinator for our Extra Care Schemes in Leeds and Wakefield.

Job Summary:

The successful candidate will be responsible for overseeing our Extra Care Schemes, ensuring the highest level of care and support for our residents. This is an exciting opportunity for someone who is passionate about delivering exceptional care services and making a real difference in people's lives.

About the Role:

This is a full-time position (35 hours per week) with a salary range of £34,600-£37,000 per annum, depending on experience, plus a £2,000 car user allowance. The initial contract duration is 6 months, with a possibility of extension.

Key Responsibilities:

  • To work across multiple extra care schemes, providing coordination and oversight of care services.
  • To collaborate with healthcare professionals, social workers, and other stakeholders to ensure seamless delivery of care services.
  • To assess and meet the needs of service users, developing personalized care plans that promote independence and well-being.
  • To monitor and evaluate the effectiveness of care services, identifying areas for improvement and implementing changes as necessary.

Requirements:

We are looking for a highly motivated and organized individual with excellent communication and interpersonal skills. The ideal candidate will have a background in care management or a related field, with experience in coordinating care services and working with vulnerable populations. A degree in a relevant field, such as social work or healthcare management, would be advantageous.
