Role Overview
Choices Homecare seeks a dedicated Care Coordinator to coordinate and deliver high-quality, user-focused domiciliary care services.
Key Responsibilities
- Coordinate and deliver a high-quality, user-focused domiciliary care service.
- Manage a team of Care Workers, ensuring excellent service delivery through performance management and staff supervision.
- Liaise with hospitals, GPs, social services, and families to ensure the best care for service users.
- Conduct assessments and reviews to maintain top care standards.
- Manage on-call duties and weekend work on a rota basis.
Requirements
- Previous experience in domiciliary care is essential.
- Strong IT skills, exceptional organisational abilities, and a passion for delivering person-centred care.
- A full driver's licence and access to a vehicle.
