Role Summary

We are seeking a highly skilled Mental Health Care Coordinator to join our team at the Leeds and York Partnership NHS Foundation Trust. This is an exciting opportunity to work in a dynamic environment and contribute to the delivery of high-quality mental health services.

About the Role

The successful candidate will have experience working with people with ongoing and mental health needs, as well as experience of formulating care plans for individuals. They will be required to coordinate the care of service users, provide comprehensive holistic mental health assessments, and develop and deliver complex care plans.

Responsibilities and Requirements
  • Coordinate the care of service users through assessment, formulation, safety planning, and clinical interventions
  • Develop and deliver complex care plans that meet individual needs and promote recovery
  • Work collaboratively with multidisciplinary teams to ensure effective care coordination and support
  • Maintain accurate and up-to-date records of service user information and progress
Person Specification
  • RN (Mental Health) qualification or equivalent
  • Experience working with people with ongoing and mental health needs
  • Experience formulating care plans for individuals
