Job Overview

Working Full Time: 37.5 Hours per Week

This is an exciting opportunity to work within the Care of the Elderly at the Countess of Chester NHS trust. As a Parkinson's Disease Nurse Specialist, you will act as a point of contact for patients experiencing problems or deterioration, providing highly specialist knowledge, skills, and advice to clients and a source of expertise to health and social care professionals.

This is an opportunity to develop your skills and knowledge in this area, working closely with the Elderly Care team to enhance the service and care pathways. You will assess individual patient needs for those living with Parkinson's to improve their quality of life by identifying, co-ordinating, and linking to appropriate care services and therapies throughout the care pathway.

Main Duties of the Job

As a member of the Elderly Care team, you will be responsible for the assessment of care needs, implementation, and evaluation of programmes of care. You will provide nursing expertise to a specific patient group and support clinical colleagues in providing a service to the PD client group.

You will work independently to support patients and their carers to maximise on the benefits of the Parkinson's Nurse Specialist role on care programmes, developing patient-focused goals, encouraging self-care, and self-management approaches.
