Primary Care Network GP

We are seeking a highly skilled and experienced GP to join our team as a Clinical Lead for our Community GP service. This is an exciting opportunity to lead a team of healthcare professionals and make a real difference to the lives of our patients.
Key Responsibilities- Provide clinical leadership to the Community GP team, ensuring high-quality patient care and effective service delivery.
- Develop and implement strategies to improve patient outcomes and service efficiency.
- Collaborate with other healthcare professionals and organizations to ensure seamless care coordination and patient-centered services.
- Lead and manage the Home Visiting Team, providing guidance and support to ensure excellent patient care.
- Contribute to the development and implementation of service improvement initiatives, ensuring alignment with the organization's goals and objectives.
- Participate in multidisciplinary team meetings and contribute to the development of care plans and strategies.
- Provide holistic and comprehensive assessments, ensuring patients receive the best possible care in their own homes.
- Maximize best care in the patient's own homes to reduce the need for hospital or care home admissions.
- Provide end-of-life care in line with patient wishes.
- Engage in quality improvement activities, identifying areas for improvement and implementing changes to enhance patient care.
- Opportunities for teaching and mentorship, supporting the development of junior staff members.
