Job Overview

Our organisation, Guy's and St Thomas' NHS Foundation Trust, is seeking a highly skilled and experienced professional to join our team as a Neighbourhood Nursing Specialist. Main Responsibilities
  • Work closely with Neighbourhood Nurses to provide advanced nursing care and support to patients with complex needs.
  • Act as a single point of contact for care, support, and coordination of services for patients with complex health and social care needs.
  • Provide holistic assessments and develop comprehensive care plans to meet the needs of patients with multiple, complex long-term conditions.
  • Prescribe medications and treatments as necessary, and refer patients to specialist services when required.
  • Work independently and as part of a multidisciplinary team to ensure seamless care coordination and delivery.
  • Provide leadership and clinical expertise to the nursing team, and participate in the development of policies and procedures related to complex care.
  • Collaborate with other agencies and services to facilitate appropriate care packages and promote independence, self-care, and wellness.
Requirements
  • Substantial experience working as a senior practitioner in a team, with a strong background in community nursing and complex care.
  • Excellent assessment and clinical skills, with the ability to demonstrate evidence-based care and decision-making.
  • Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
  • Post-registration education and training in the care of older people and long-term conditions, with a teaching certificate or equivalent mentorship training.
  • Advanced assessment and long-term conditions skills, with a nurse formulary prescriber qualification and independent prescribing module completion or willingness to undertake.
