As a key member of our community nursing team, you will be responsible for providing advanced nursing care to patients with complex needs, working closely with our Neighbourhood Nurses and other healthcare professionals to ensure seamless care coordination.
Main Responsibilities- Carry out comprehensive assessments of patients with complex needs, using advanced level clinical skills to identify and address their health and social care requirements.
- Develop and implement individualised care plans, working collaboratively with patients, carers, and other healthcare professionals to ensure effective care delivery.
- Provide expert clinical guidance and support to Neighbourhood Nurses, promoting best practice and ensuring high-quality care standards.
- Lead and participate in multidisciplinary team meetings, case conferences, and other key meetings to ensure effective care coordination and patient outcomes.
- Contribute to the development and implementation of policies, procedures, and guidelines related to complex care, ensuring they are evidence-based and effective.
- Participate in clinical audit, benchmarking, and evaluation activities to monitor the impact of the Community Nursing role on clinical and cost-effectiveness.
- Develop and maintain strong relationships with patients, carers, and other healthcare professionals, promoting effective communication and collaboration.
- Stay up-to-date with the latest clinical guidelines, research, and best practices, applying this knowledge to improve patient care and outcomes.
- Substantial experience working as a senior practitioner in a team, with a strong background in community nursing and complex care.
- Excellent assessment and clinical skills, with the ability to work autonomously and make informed decisions.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
- Ability to lead and implement successful change management, promoting best practice and high-quality care standards.
- Post-registration education and training in the care of older people and long-term conditions, with a teaching certificate (ENB 998/997) or equivalent mentorship training.
- Advanced assessment, long-term conditions, and nurse formulary prescriber qualifications, with independent prescribing module completion or willingness to undertake on taking up post.
