About the Role

We are seeking an experienced and skilled Neighbourhood Nursing Specialist to join our team. As a key member of our community nursing team, you will be responsible for providing high-quality care to patients with complex needs, working closely with our multidisciplinary team to ensure seamless care coordination.

Key Responsibilities
  • Provide advanced nursing care to patients with complex needs, including disease treatment, prescribing, and palliation.
  • Work closely with our multidisciplinary team to ensure seamless care coordination and promote patient-centered care.
  • Develop and implement individualized care plans, taking into account patients' unique needs and preferences.
  • Collaborate with other healthcare professionals to ensure effective communication and coordination of care.
  • Participate in clinical audits and quality improvement initiatives to ensure high-quality patient care.
Requirements
  • Registered Nurse (General Adult) with a strong background in community nursing.
  • Post-registration education and training in the care of older people and long-term conditions.
  • Advanced assessment and long-term conditions skills.
  • Non-medical prescribing qualification or willingness to undertake.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
