Creative Support is a leading provider of adapted supported living services. Our Chorley ABI Service provides a bespoke model of support that enables individuals to live fulfilling and valued lives.
Job Description:As a Senior Support Worker, you will be responsible for providing person-centred support to an individual living in a hospital setting who is moving into our new adapted supported living service. Your key responsibilities will include:
- Developing and implementing a person-centred care plan that addresses the individual's needs and goals.
- Working collaboratively with the individual, their family, and other professionals to ensure a smooth transition.
- Providing emotional and practical support to the individual to enable them to lead a fulfilling and valued life.
- Experience working with individuals with challenging behaviours.
- Ability to work independently and as part of a team.
- Good communication and interpersonal skills.

- A salary of £28,000 - £35,000 per annum, depending on experience.
