We are seeking a skilled and experienced Mental Health Liaison Practitioner to join our dedicated team at the James Paget University Hospital in Gorleston. As a key member of our Mental Health Liaison team, you will play a vital role in supporting individuals presenting to the acute hospital with mental health issues.
Key Responsibilities- Provide effective risk assessment and risk management to ensure the safety and well-being of patients.
- Work closely with acute hospital colleagues to create a proactive interface between mental health and acute services.
- Support individuals presenting to the acute hospital with mental health issues, including those with new or ongoing mental health difficulties and illness.
- Contribute to timely and supportive discharge planning to ensure patients receive the necessary support and care.
- Nursing degree or equivalent, DIP OT, Clinical Psychology or equivalent.
- Minimum 1 year experience at B6.
- Competent assessment skills.
- Competent in cognitive assessments.
- NHS pension.
- Comprehensive in-house and external training programs.
- Career progression.
- Starting annual leave of 27 days per annum increasing to 33 days pa based on length of service (plus paid bank holidays).
