We are seeking a skilled and experienced Mental Health Liaison Practitioner to join our dedicated team at the James Paget University Hospital in Gorleston.
As a key member of our Mental Health Liaison team, you will play a vital role in supporting individuals presenting to the acute hospital with mental health issues.
Key Responsibilities- Conduct effective risk assessments and develop proactive risk management strategies
- Collaborate with acute hospital colleagues to ensure seamless care pathways
- Provide expert mental health support and guidance to individuals and their families
- Contribute to timely and supportive discharge planning
We are looking for a motivated and compassionate individual with a nursing degree or equivalent, and experience in acute mental health and risk management.
Competent assessment skills and experience in cognitive assessments are highly desirable.
