We are seeking a dedicated and empathetic Social Worker to join our Older People & Hospital Team. The successful candidate will work with multi-disciplinary teams to ensure smooth and safe discharges for older adults transitioning from hospital care to community living.
Key Responsibilities:
- Conduct assessments, develop care plans, and provide interventions for older adults.
- Work with healthcare professionals, discharge coordinators, and community services to ensure smooth discharges.
- Provide ongoing support to older adults and their families, promoting independence and well-being.
- Advocate for older individuals, ensuring their needs are met through appropriate care and services.
