Job Overview

We are seeking a highly motivated and effective Community Nurse to join our team at Buckinghamshire Healthcare NHS Trust. As a Community Nurse, you will work in a specialized area of practice, providing high-quality nursing care to patients in their own homes and in residential care settings.

Main Responsibilities
  • Provide comprehensive nursing care to patients with complex health needs, including medication administration, end-of-life care, and wound management.
  • Work collaboratively with a multidisciplinary team, including District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners, and Healthcare Assistants.
  • Develop and implement individualized care plans, taking into account the patient's physical, emotional, and social needs.
  • Conduct thorough assessments, identify risks, and develop strategies to prevent hospital admissions.
  • Communicate effectively with patients, relatives, carers, and other healthcare professionals to ensure seamless care coordination.
Requirements
  • Registered Nurse or student nurse awaiting confirmation of NMC registration.
  • Experience in relevant clinical areas, such as Care of Elderly, Community, Orthopaedics, Neurology, Respiratory, Palliative Care, and Long-term Conditions.
  • Ability to work under pressure, manage own caseload, and prioritize tasks effectively.
  • Knowledge of the NMC Code of Professional Conduct and the Data Protection Act.
  • Car driver/owner able to travel to other locations within the trust.
