Main Responsibilities- Work with GP practices across Derbyshire to deliver prescribing-related cost-efficiencies in line with agreed standard operating procedures.
- Provide education, advice, and support to practices and care homes to improve the safety, quality, and cost-effectiveness of patient care.
- Retrieve, analyse, interpret, and present patient-related prescribing and clinical data from practice clinical computer systems and written notes.
- Implement prescribing changes and inform patients, practice staff, and local communities.
- Deliver education to healthcare professionals, care home, and social care staff, patients, and the public to promote effective medicines use.
- Liaise with GPs, secondary care, community social care, community pharmacy, patients, and carers to deliver medicines optimisation.

Requirements- Essential:
- Education to degree level in a relevant subject or equivalent level of experience working at a similar level in a specialist area.

- Registered with the General Pharmaceutical Council (GPhC).
- Desirable:
- Experience of working with care homes.
- Experience of primary care.
