About the Role

We are seeking a skilled and compassionate Rehabilitation Support Worker to join our integrated Health and Social Care community rehabilitation service in Eastbourne, covering the Eastbourne Hailsham & Seaford area.

Job Summary

The successful candidate will work as part of a friendly, supportive, and professional team to provide rehabilitation services to individuals in their own homes, promoting independence and quality of life.

Main Responsibilities

  • Key working with non-complex clients to provide rehabilitation and support
  • Responding to urgent referrals to support clients to stay in their own homes
  • Liaising with clients in acute and intermediate care settings prior to discharge

Requirements

  • GCSE grade 4 or GCE C and above
  • Level 2 functional skills or healthcare support worker apprenticeship/NVQ level 2 in a relevant subject
  • Level 3 apprenticeship or NVQ level 3 in a relevant subject or equivalent level of knowledge and experience
  • Experience in health or social care settings
