About the Role
We are seeking a skilled and compassionate Rehabilitation Support Worker to join our integrated Health and Social Care team in Eastbourne, covering the Eastbourne Hailsham & Seaford area.
Job Summary
The successful candidate will work as part of a friendly, supportive, and professional team, providing rehabilitation support to individuals with a variety of conditions in their own homes. The role involves key working non-complex clients, providing rehabilitation, and responding to urgent referrals to support clients to stay in their own homes.
Main Responsibilities
- Provide rehabilitation support to individuals with a variety of conditions in their own homes.
- Key work non-complex clients and provide ongoing support.
- Respond to urgent referrals and support clients to stay in their own homes.
- Work flexibly and visit clients in their own homes and liaise with clients in acute and intermediate care settings if necessary prior to discharge.
Requirements
- GCSE grade 4 or GCE C and above.
- Level 2 functional skills or healthcare support worker apprenticeship/NVQ level 2 in a relevant subject.
- Level 3 apprenticeship or NVQ level 3 in a relevant subject or equivalent level of knowledge and experience.
- Experience in health or social care settings such as employment, work experience, volunteering, or placements.
